Keep it short for print. Every extra field makes the code denser. For a business card, the name, one phone number, an email and a website scan best. Leave the note out if the code looks crowded.
Test before you print. Scan it with both an iPhone and an Android phone. iPhones are stricter about contact formatting than Android.
